Every Care Plan Has a Person Behind It
Why documentation must never replace understanding

A care plan can describe a person’s needs. It cannot contain the whole of their life.
The document is not the person
Care plans protect continuity and safety, but the written description is never the complete reality. A person is not a diagnosis, risk or task.
A life existed before the plan
The person receiving support may once have led a household, raised a family, built a career or served a community. Current dependency does not erase identity.
Preferences are dignity
A preferred name, familiar music, food, faith and routine may look small from the outside. They can provide continuity and control.
Keep the plan alive
People and needs change. Good plans evolve through observation, family insight, professional review and the person’s own voice.
Reflection
The record supports the care. The person gives the care its meaning.
